(2016). Centers of Disease control and Prevention (CDC) defines SDOH as “Conditions in the places where people live, learn, work, and play affect a wide range of health risks and outcomes,” (CDC, n.d.b). The family SDOH, health screening, and family health promotion model have executed, and this assessment contains the plan of action concluded from those. SDOH and Its Impacts on Family Based on the assessment of Mr. Koshy and family, SDOH falls under the categories of social environment, physical environment, and health services. The SDOH affecting this family are culture, family traditions and beliefs, marital status, lifestyle, impact of migration, adaptation of a new culture and health insurance. This is an Indian family who has migrated to the U.S almost eleven years back, continuously trying to adapt to American culture. The parents are in their late forties and are greatly influenced by the Indian culture and family traditions, which makes Mr. Koshy, the decision-making figure in the family.
