Referral: Mental health assessment Urine drug screen Institute close observation; suicide and self-harm precautions (Per hospital/unit policy/procedures) 1:1 sitter (This can be delegated to trained non-professional staff and supervised by RN) Lorazepam 1 mg by mouth as needed every 2 - 4 hours. Max dose 8 milligrams per 24-hour period Determine mental health of client and assess for further complications or other problems in relation to client’s mental health. Urine drug screen to rule out any substance abuse or any other substances that could be triggers or contribute to client’s mental health Close observation to keep the patient safe and monitor for any suicide ideation or attempts as well as self-harm attempts Monitoring for close observation, suicide, and self-harm precautions. Client will have an individual that will be able to intervene and warn others if anything were to happen Benzodiazepine that is approved for anxiety and can help to decrease anxiety that the patient is feeling. This will also hope to decrease the suicidal ideation Identification of other complications and assessing client’s mental health Urine drug screen will be negative Client will be monitored, and any complications avoided.
