NURS 615 Assignment 13.2 Shadow Health Focused Exam Community Acquired Pneumonia. The shadow health assignment for this week was about Dr. Anita Douglas, who is 72 years old female who came to th... e clinic with a three-day history of cough, shortness of breath, fever, nasal congestion, increasing fatigue, and chest pain which got worse with coughing. She did take some aspirin to relieve her symptoms, but it did not help. Dr. Douglas reported the colored sputum and increased respiratory symptoms. She denied any recent hospitalization, no smoking but drank occasionally, and compliant with her home medications. She walks two to three times a week around her neighborhood. She also complained about increasing physical discomfort and not able to walk like before. She is pretty good in her diet in general and increases the liquid food during this period, such as soup, water, and tea. She had a medical history of hypertension and chronic kidney disease, which she was diagnosed with a while back. Therefore, she takes trandolapril/ verapamil and chlorothiazide at home. In addition to that, she had a family history of hypertension. With all the signs and symptoms that I learned about Dr. Douglas, I determined she must diagnose community-acquired pneumonia. Community-acquired pneumonia is prevalent and will lead to severe effects among the elderly if not appropriately treated. This assignment helped me learn more about CAP and various medications that can be used and the side effects. After keeping an eye on her comorbidities and some research, I decided to prescribe her Augmentin, the combination medication of amoxicillin and clavulanic acid, and doxycycline. Augmentin 875/125 mg every 12 hours for seven days and doxycycline 100 mg every 12 hours for five days. I gave her some information about the side effects such as rashes, shortness of breathing, body swelling, sun protection, and informing the health care provider if she noticed any allergic reaction. I instructed her about completing the total dosage of medications even if she started feeling better with few dosages to prevent the possibility of antibiotic resistance. References Woo, T.
