In Heart attack or myocardial infarction, the most common ECG changes would be the ST elevation or depression of the PR segment. The position or the indicator which artery is affected depends on what leads were affected. The most common site of MI is the LAD, hence an ST elevation on lead I, and aVL can be seen. There are instances wherein there are different arteries are blocked, hence the lead changes. Troponin I elevates due to cardiac injury, therefore in MI, it is expected to rise.
