She reports rash eruption about 10days ago with itching, painful urination and genital ulcer. She reports that the rash is accompanied by fever and headache. She stated that oral temperatures for the past 3days has been ranging from 99- 100.4 F. She reports reluctance to urinate because of pain on urinating. She rated her pain level 6/10, constant, and worst on walking. She reports doing sitz bath in her tub with no relief. She also used Tylenol 650 mg twice a day for the pain. Medications: (list with the reason for med ) Tylenol 650 mg oral 6 hours as needed for pain and fever. Sprintec (norgestimate 0.25mg and ethinyl estradiol 35mcg) PO once daily for 21 days, followed by 1 inactive tablet PO once daily for 7 days. PMH: None Allergies: seasonal allergies, NKA.
