As a nurse I had to make sure I was there for the family and help them understand the best way to transition through this major life event of Lucy losing her life partner and Neil losing his mother. What nursing actions did you feel were appropriate within this scenario? After the patient passed away I made sure the family was aware of various ways on how to handle a passing of a loved one and explained to them that if they were not sure how to feel, oh what were they feeling to reach out and ask for help because we are there to help them. I also made them aware of support groups in the community that may be able to help go through grief of losing their loved one. Scenario Analysis Questions* PCC What priority problem(s) did you identify for Julia Morales and Lucy Grey? Lucy was concerned about Julia being unresponsive and about her breathing not being good. PCC/I What is the importance of therapeutic communication during the dying process and postmortem care? It ensures that the family is aware that we are here for them, with anything they need, whether is for them to cry on our shoulder, vent and we sit with them and listen. PCC What potential barriers might affect Julia Morales’s wish to die peacefully? Often dying patients feel like they still have unfinished business, like Julia felt like she was leaving her partner too soon and she still had more travel plans together but it looked like they were not happening which made her sad and feels like she left Lucy. PCC/I What strategies might empower Lucy Grey in making decisions about Julia Morales’s end-of-life care? Family education was one of the things that was done properly in this scenario while Julia was alive which in fact eased thing on Lucy and Neil. Family education was properly done so the family and the patient did take the proper steps to make sure everything was done they don’t have to deal with these things after Julia’s passing. PCC What can the health care team members caring for Julia Morales do to cope with their grief over the patient’s death? Find out more about their spiritual/ religious beliefs and make sure to direct them to the proper route which will help with grief. This could be bringing a pastor to pray with the family, support groups, or whichever way they used in the past to get over a loss or hard times. T&C What is the role of the home health/hospice nurse and interprofessional team after Julia Morales’s death? How was the situation-background-assessment-recommendation (SBAR) technique used to communicate with the primary care provider? • To be there for the family of the patient who passed away, offer support. Situation Patient Julia Morales 65, Stage 4 lung cancer passed away peacefully in the presence of her partner and son. Patient’s family and son are still bed the patient’s bedside and are sad that Julia is gone. Background Julia was treated for lung cancer with chemo and radiation but wished to stop the treatments. She lived with her partner Lucy 73 for 25 years who is here with her along with Julia’s son Neil. Assessment Julia is not responsive and her breathing keeps declining. Recommendation Keep her as comfortable as possible, pain free and adequate oxygenation. S What measures can ensure Lucy Grey’s safety after Julia Morales’s death? It will be important to assess Lucy’s living situation and social support when it comes to friends and family. Assess mental health to identify how does she see life after Julia’s passing. Identify any trigger of anxiety and refer her to the proper health care team to get proper care.
